The Nutrigenomix client consent and Daisy Fields Company | EatRight-LiveWell terms documents are REQUIRED. If you prefer, ask us to email the forms to you. Contact us via email or book a 15 minute complimentary call with Tam.

Please complete and sign both (initial all pages and sign the ER-LW terms) and return as a PDFs with your order email indicating which test you are ordering. Include the mailing address (no PO Box delivery) where you want the test shipped. Please be specific for multiple tests in one order. We will follow your directions precisely. Include a phone number for delivery and in case we have shipping questions. Send the completed documents to EatRight-LiveWell. We will send you a business invoice and secure payment link. If you prefer a bank check payment let us know and you can mail it. Once the order is placed we will email you a paid business invoice/sales receipt.

Tests ship two day priority, are ordered on the fourth business day after payment is received to allow you, the client, time to cancel (three business days) should you change your mind. On the fourth business day, the order is NOT REFUNDABLE (see Terms agreement). The packaging and postage with detailed directions is included in your shipment. The Nutrigenomix laboratory sends EatRight-LiveWell the test PDF (we email to you) two weeks after they receive your sample. Your magazine report will be mailed to you.

Note from Nutrigenomix about privacy of your data: Each sample collection kit is marked with a unique barcode so no personal identifying information is stored in our facilities. All genetic data is sent through a secure 256-bit encryption server. The Nutrigenomix lab stores anonymized DNA samples only for future use when new test panels become available, so that clients have the opportunity to purchase a new test at a discounted rate (since we would not have to collect another sample). A client can always request to have their sample and data destroyed if they wish.

Nutrigenomix will never use client data for research purposes or reveal information or genetic data to a third party, except as required to provide the services requested, or as required by law. In addition, Nutrigenomix cannot be used to determine paternity, identify relatives or identify an individual (e.g. for forensics).

Nutrigenomix does not test for genetic predisposition to certain diseases. The test only includes genes related to nutrition and physical activity.
