The Daisy Fields Company | EatRight-LiveWell terms document is REQUIRED. Please complete (initial all pages and sign the ER-LW terms). Return as a PDF with your order email. Others may not ask for this, but when it comes to getting you what you want, we don’t take short cuts. Include the mailing address where you want the test kit shipped. Please be specific for multiple tests in one order. We will follow your directions precisely. Include a phone number for delivery and in case we have shipping questions. We will send you a business invoice and secure payment link. If you prefer a bank check payment let us know and you can mail it. Once the order is placed we will email you a paid business invoice/sales receipt. Plus you will have our phone to call us directly with questions.
Test kits ship USPS, and are mailed on the fourth business day after payment is received to allow you, the client, time to cancel (three business days) should you change your mind. On the fourth business day, the order is NOT REFUNDABLE (see Terms agreement).
The packaging and postage with detailed directions is included in your test kit shipment. Trace Elements laboratory sends us the test PDF (we email to you) after your sample is received and analysis complete.
You must send your hair sample and completed form to the lab within 60 days of ordering the test. Sample kits not received at the lab within 60 days will be subject to an increase in pricing if applicable. Contact us to discuss if this situation applies to you.
Laboratory response times vary from a couple of weeks to longer depending on their volume of tests. Feel free to check in with us about the status of your test results. In no circumstances is the test cancellable or refundable.
